Non-discrimination and accessibility

You belong here. SOS Wellness LLC provides care that is accessible, welcoming, and respectful of who you are, with free language assistance and accommodations on request.

Our non-discrimination statement

SOS Wellness LLC complies with applicable federal and Ohio civil rights laws and does not discriminate, exclude people or treat them differently on the basis of race, color, national origin, ethnicity, religion, age, disability, sex, sexual orientation, gender identity or expression, marital or family status, veteran status, genetic information, or source of payment, including Medicaid.

This includes pregnancy and parental status, neurodivergence, and any other characteristic protected under federal, state, or local law. The statement applies to every part of our services: clinical care, communication, appointment scheduling, billing, telehealth, and every interaction with our team.

Free language assistance

If English is not your preferred language, free interpreter services are available for your appointments and for conversations with our office. Written materials can be provided in other languages and in alternate formats such as large print or screen-reader-compatible electronic documents, at no cost to you.

For accessibility accommodations or free interpreter services, contact us at 513-928-3339 (TTY: 711).

  • Clear communication and written materials provided when helpful
  • Flexible appointment formats and environment accommodations
  • Self-regulation tools such as fidgets and headphones are welcome
  • Advance notice of schedule or provider changes whenever possible
