Serving Greater Cincinnati and Butler County from our Middletown clinic.
Recovery is not one-size-fits-all. Our Intensive Outpatient Program (IOP) brings psychiatry, therapy and addiction treatment together in one coordinated program, so you get structured support while you keep up with work, family and daily life.
IOP is ASAM Level 2.1 (intensive outpatient): 9–12 hours of structured programming each week, for adults 18 and older who need more than weekly appointments but not 24-hour care.
Is IOP right for you?
Our IOP serves adults who need structured treatment for substance use, often alongside depression, anxiety, PTSD, bipolar disorder, ADHD or OCD, but who do not need residential care. It may be the right step if:
- You are stepping down from residential treatment, detox or a hospital stay
- Weekly therapy is not giving you enough structure or support
- You are ready for group-based recovery and motivated to stay sober
- You need to balance treatment with work or family responsibilities
- You have been asked to complete an IOP for a court or diversion program
Not sure? Call 513-928-3339 and our intake team will talk it through with you. A clinical assessment confirms the right level of care before you start.
What is included
Each week combines group therapy, individual therapy and medication management, coordinated by one team.
- Group therapy with a small, consistent group led by a licensed clinician
- Weekly individual counseling: 50-minute one-on-one sessions with your counselor
- Psychiatric evaluation and medication management
- Medication-assisted treatment (MAT) when clinically indicated, prescribed in-house
- Treatment for co-occurring depression, anxiety, PTSD, bipolar disorder, ADHD and OCD
- Skills from CBT and DBT for coping, emotional regulation and relapse prevention
- Care coordination for practical barriers such as housing, transportation, benefits and community resources
- Family involvement when clinically appropriate
Relapse prevention is not a module at the end of the program. It runs through every session from the beginning, so you build coping strategies, emotional regulation skills and the self-awareness to notice early warning signs.
Medication-assisted treatment (MAT)
FDA-approved medications are prescribed and monitored by our own clinical team, alongside counseling. You do not need a separate prescriber.
For opioid use
- Buprenorphine (Suboxone)
- Naltrexone (oral, or a monthly Vivitrol injection)
For alcohol use
- Naltrexone (oral, or a monthly Vivitrol injection)
- Acamprosate (Campral)
- Disulfiram (Antabuse)
What MAT includes
- Medical evaluation
- Dosage monitoring
- Side effect management

Getting started
Starting takes one phone call. Here is what happens next.
Step 1: Call us
Call 513-928-3339, 7 AM–11 PM, or start on our appointments page. A member of our intake team will register you, help you set up your patient portal and schedule your first appointment with a therapist.
Step 2: Clinical assessment
Your therapist conducts a clinical assessment to confirm that IOP is the right level of care and begins building your individualized treatment plan.
Step 3: Insurance verification and care coordination
We verify your benefits before your first session so you know what to expect. You will also meet a care coordinator who helps with any practical needs or barriers and connects you with community resources.
Step 4: Start the program
Once your group is ready, your therapist schedules your start date at our Middletown clinic.
Step 5: Ongoing care
As you complete IOP, your clinical team moves you into a structured step-down plan: continued individual therapy, group check-ins and ongoing medication management when needed. Support continues well beyond the program.
Your care team
Your care is directed by our Medical Director, a psychiatrist, and provided by one consistent team that includes psychiatric mental health nurse practitioners, licensed social workers and case management staff. Treatment uses approaches supported by research and clinical evidence, including CBT, DBT and trauma-informed care.
Insurance and access
We accept Ohio Medicaid managed care plans, Medicare and many commercial plans, including Anthem Blue Cross Blue Shield, UnitedHealthcare, Cigna, Aetna, Medical Mutual of Ohio and Humana. Coverage depends on your plan and service; our team verifies benefits before your first visit.
If you are paying for care yourself, ask about self-pay options and payment arrangements. Our billing team can discuss expected costs before you schedule.
Quality and accreditation
SOS Wellness holds CARF Preliminary Accreditation and is licensed by the Ohio Department of Mental Health and Addiction Services (OhioMHAS). Our intensive outpatient treatment program for integrated substance use and mental health care is one of the programs included in that accreditation.

Common questions
Answers to the questions we hear most often. If yours is not here, call 513-928-3339 and our intake team will help.
What is the difference between outpatient care and IOP?
Outpatient care means individual appointments, weekly, biweekly or monthly, for medication management and therapy. IOP provides 9–12 hours of structured programming per week with group therapy, individual therapy and medication management, for people who need more support while living at home.
Do you treat mental health conditions alongside addiction?
Yes. We treat co-occurring conditions together, because they affect each other. Our psychiatric team treats depression, anxiety, PTSD, bipolar disorder, ADHD and OCD alongside addiction recovery.
What happens after I finish IOP?
We help you transition to continuing care, whether that is weekly therapy, support groups such as AA or SMART Recovery, or ongoing psychiatric care. Your case manager makes sure there is no gap in support.
How do I know if IOP is the right level of care?
During your assessment we help determine the right level of care. IOP is usually a good fit when weekly therapy is not providing enough structure, or when you are stepping down from residential treatment.
Can I complete IOP from home?
Yes, through our Virtual IOP, which follows the same 9–12 hour weekly structure by secure telehealth. You must be physically located in Ohio during each session.
