Portrait of Kristen Dzubak, QMHS

Kristen brings more than 12 years of experience in the mental health field, with a career dedicated to supporting individuals and families as they navigate complex mental health needs. She holds an Associate of Applied Science degree and is a Certified Medical Assistant. For nearly three years, Kristen has focused her career specifically on case management, where she is passionate about helping clients overcome barriers, connect with resources, and work toward greater stability and independence. As a Case Manager, Kristen provides comprehensive support to individuals with a variety of psychiatric needs. Her experience includes crisis intervention, symptom monitoring, medication and treatment coordination, SSDI assessments, and Expert Evaluations for Guardianship. She also works closely with clients involved with the court system, including those under probate, providing ongoing monitoring, advocacy, and coordination to help ensure their needs are appropriately addressed. Kristen believes effective case management often requires collaboration beyond the mental health setting. She works closely with members of the care team as well as schools, courts, law enforcement, probation officers, Child Protective Services, hospitals, pharmacies, and community organizations. Through these partnerships, she helps connect clients with appropriate services and resources while advocating for individualized care and the best possible outcomes. For seven of her 12 years in mental health, Kristen also specialized in neurology, giving her additional experience working with individuals whose mental health needs intersect with neurological conditions. This background has strengthened her ability to approach each client as a whole person and recognize the many factors that can influence their overall well-being. Kristen is known for being compassionate, dependable, and persistent in her work. She approaches each client with patience, respect, and genuine care, believing that every person deserves to be heard, treated with dignity, and given the opportunity to succeed. She has a strong passion for helping others and takes pride in being someone her clients can count on when navigating difficult circumstances. Outside of her professional life, Kristen deeply values her faith and family. She strives to carry those values into her work and daily life by treating others with kindness, compassion, and grace. Whether she is advocating for a client, helping someone navigate a difficult situation, or simply offering encouragement, Kristen believes that even small acts of kindness can make a meaningful difference.
