Your application

SOS Wellness collects the contact information, professional details, cover note and résumé you submit to assess your candidacy and contact you about employment. Applications and files are encrypted in storage and available to authenticated SOS website administrators in a separate recruitment queue.

Please do not include patient information, medical records or sensitive health details. This form is not for appointment requests or clinical care.

Optional advertising measurement

You can choose whether Google Ads measures your completed job application. The choice is off by default and does not affect your application. If you agree, the confirmation page loads a Google advertising tag, which can use advertising cookies, your ad-click identifier, browser/device information and IP address to attribute the submission. The event includes an application transaction identifier to prevent duplicate counts.

Your name, contact details, résumé, cover note, license details and job title are not included in the conversion event. We do not use enhanced conversions or personalized advertising for this recruitment measurement. Patient appointment submissions are not sent through it.

If you decline, this recruitment flow does not load the Google advertising tag. Your choice applies to this application. Google handles information it receives under its privacy policy.
